Driver's Safety Certificate and DL Records Submittal

This form allows you to submit:

  1. Your Driver's Safety Course Completion Certificate (Must Have Texas Department of License and Regulation Logo). The Court (top portion) Copy MUST be signed before it can be accepted.
  2. Your Texas 3A Certified Driving Record (Must Have Department of Public Safety Seal). *May not be required if doing Deferred Disposition
  3. Please allow 3-5 days for the court staff to process your submission. You may also check your case status online by clicking here. If you are taking a Driver's Safety Course for a Deferral of Final Disposition, you may also use this form.

It is the Defendant's responsibility to ensure all requirements of the order are satisfied and submitted properly.

Is your driver's safety course certificate of completion signed and does your driving record state it's a Certified Driving Record? If not, we cannot accept it until the court copy is signed and you have the correct driving record.
You must attach a scanned or PDF copy of your Driver's Safety Course Certificate of Completion bearing the TDLR logo with your signature. The Double Oak Municipal Court(or variation of) must be listed on the Court Copy of the certificate
